正常成人每天合成的免疫球蛋白為2～5g[1]。但是在受到感染以后，其的合成量可增加7倍左右[2]。因此，對Ig的測定可以作為檢測多種疾病的依據，同時在臨床上，還將其作為臨床診斷和觀察以及預后的重要觀察指標。然而，對血清Ig的檢測結果也同時受到多種因素的影響，比如檢測儀器；標本處理；操作技術；實驗試劑等。在同一實驗室的不同批次或不同實驗室中對其進行檢測，也會出現一定的差異。但是，在國內對血清標本的放置時間和保存溫度對IG的檢測結果存在的影響，報道的非常少。本文采用全自動生化檢測儀器對不同溫度和時間對血清進行保存，并對Ig進行檢測，主要是探討臨床上保存血清的最佳條件。以下是詳細報道。

1 資料和方法

1.1 一般資料 我校在2012年6月～2013年10月期間，一共選擇了200名健康的體檢學生。都是男性，年齡在22～30歲之間，平均年齡為（27.5±2.0）歲。他們都沒有感染性和系統性的疾病，同時沒有出現肝腎和心血管等疾病。

1.2 方法

使用干燥的試管對他們進行靜脈抽血，大約為5ml。在室溫下放置1h，使用3500rpm進行離心10min，最后去最上層的血清，分開裝到5根試管中。分別放置到室溫和4℃以及-20℃的環境中進行保存。在室溫下進行保存的血清，在保存2h后，對其進行檢測IgA、IgG、IgM。保存在4℃和-20℃的血清，分別在第3d和第6d進行檢測。IgA、IgG、IgM的定量測定試劑盒由上海科華生物工程股份有限公司生產。用Hitachi 7180全自動生化分析儀（日立公司生產）進行免疫比濁法測定，所有操作均嚴格按照試劑盒說明書進行[3]。

1.3 統計學方法 對所有的數據都使用均數±標準差（x±s）來進行表示[4]。并使用SAS15.0軟件進行統計和分析。并采X2檢驗。差異顯著具有統計學意義（P

免疫比濁法是目前臨床上測定血清IgA、IgG和IgM的主要方法，包括透射比濁法和散射比濁法。兩種方法的測定原理和儀器不同，其結果有所差異。透射比濁法主要檢測抗原抗體復合物所形成的濁度，用全自動生化分析儀檢測；散射比濁法是從不同角度測量抗原抗體復合物微粒的散射光強度和濁度的變化，用蛋白分析儀檢測。一般認為散射比濁法的靈敏度高、重復性好，但國內研究顯示散射比濁法和透射比濁法準確性、精密度均比較好。但散射和濁法和透射比濁法測定Ig的結果，發現偏差均在可接受范圍，變異系數均小于3%，說明二者均為IgG、IgA、IgM的可靠測定方法。全自動生化分析儀的應用，使Ig的檢測速度大為加快。如果使用性能較好的全自動生化分析儀及相應的配套試劑，就能保證IgA、IgG和IgM透射比濁法測定結果與散射比濁法一致。日立7180全自動生化分析儀具有處理能力大、測試速度高、標本用量小等優點，在精密度、線性、準確度等方面性能良好。

血清標本的分離與保存、檢測儀器和試劑、操作技術等因素均對Ig檢測結果有影響，在檢測過程中必須做好各個環節的質量保證措施，減少或消除檢驗誤差。用全自動生化分析儀測定血清樣品，新鮮標本較好。由于免疫比濁法測定的是溶液濁度，故脂血、溶血標本對測定結果影響很大。適宜的溫度可加快抗原與抗體結合，從而加速了濁度反應。在0～56℃之間，溫度越低，反應越慢，但反應生成的抗原抗體復合物結合致密、牢固，易于觀察；溫度越高，反應越快，但反應生成的抗原抗體復合物不穩定，易解離。溫度過高（>56℃）可能引起抗血清球蛋白變性及某些血清球蛋白破壞。因此，多數抗原抗體反應以37℃為宜。

經過實驗表明，在4℃下對血清進行保存3d，IgA的含量出現了明顯性的變化。然而IgG在6d時也出現了很明顯的升高情況[6]。但是IgM卻沒有出現變化。在-20℃條件下對血清進行保存3～6d時，IgA和IgG以及IgM的含量都沒有出現明顯性的變化。在4℃時對血清進行保存時，IgA和IgG的含量升高，然而IgM的含量卻沒有變化，主要是因為血清中的IgA、IgG大多是單體。IgA和IgG的分子肽鏈相對于IgM來說，更容易被血清中的蛋白酶水解掉各種的片段。然而這些被水解的片段又會與試劑中的相應的抗體進行結合。研究結果表明：在4℃下保存6d的血清不適合進行Ig的檢測。但是在-20℃下保存的血清卻可以進行Ig的檢測。
